Dear faculty member;

The CSC office is compiling a Consultancy Directory for KFUPM faculty within their specialties and expertise to be available for consultancy services for the industry. If you are interested to be in the directory please read the consultancy policy and fill the information format below;

Form-A: Faculty Application.

Form-B: This form is to be filled and submitted, if you have a consultancy request and to be approved by the department Chairman and CSC office.

Form-C: This form is to be filled upon completion of the consultancy job and to be submitted to the CSC office.                        

Form-D: Contract Format.
